Payment Options
The Clinicians at the CDCW are paneled on numerous insurance and managed care panels. Please contact us for the specifics on any clinician. Among the insurances accepted are the following:
- Aetna (PPO and HMO)
- Blue Cross/Blue Shield PPO (and certain BC/BS HMO plans)
- Department of Children and Family Services (Psychological/Neuropsychological Assessment)
- Employee Resource Center (ERC)
- Evernorth/Cigna
- First Health
- Humana (now Lifesource)
- Magellan Behavioral Health
- Medicare
- Preferred HealthCare (PHCS)
- United Behavioral Health (UBH)
- Value Options (VBH)
CDCW processes payments in any of the following forms:
- Cash
- Credit (Visa, Mastercard, Discover, Amex)
- Check
When a provider is contracted by your insurance, the fee schedule is governed by the contract, including your co-insurance or co-pay rate and deductibles. What is considered our “Usual and Customary Fee” for service rate is as follows:
- Initial Intake Session – $250.00
- Individual Therapy Session – $200.00/hr
- Family Therapy Session – $210.00/hr
- Group Therapy and Workshop Fees – Varied by Group
- Medication Management – Initial Intake Session – $250.00
- Medication Management – Routine Monitoring Visit – $150.00
Self-pay Fees
- Initial Intake Session – $150.0
- Individual Therapy Session – $120.00/hr
- Family Therapy Session – $130.00/hr
- Group Therapy and Workshop Fees – Varied by Group
- Medication Management – Initial Intake Session – $200.00
- Medication Management – Routine Monitoring Visit – $100.00
CDCW does provide a Sliding Fee Scale for Clients, depending on the need and for those who meet criteria for the state of Illinois.
We also train Doctoral Level Psychology Students who can provide services at significantly reduced rates. Please inquire with us regarding these arrangements.
